The best way to get Baneful Hearts in Diablo 4 Season 4 is by opening Tortured Chests in Helltides. These have a fairly high chance of dropping Baneful Hearts, though you will need 100 Aberrant Cinders to open them in the first place.
Thankfully, you can gather those quickly by slaying enemies in Helltide.
You can also get Baneful Hearts by killing Hellborne bosses. These are powerful enemies that only spawn in Helltide after your threat level has reached 3, and you get hell-marked.
Your threat level will reset after the Hellborne spawns, so make sure you’re well-prepared to tackle it, or else you have to wait until your threat level reaches 3 to face it. We recommend playing in co-op if possible.
Thankfully, as of this writing, the drop rate of Baneful Heart from Tortured Chests and by killing Hellborne are pretty high, so you should be able to gather a handful of those quickly.
You can check the amount of Baneful Heart you currently have on the right-hand top side of the screen, next to the Aberrant Cinders.
You will need at least three Baneful Heart to use the Accursed Ritual and summon the Blood Maiden, who is essentially the main boss of this season’s Helltide. Naturally, it’s a tough encounter, but killing the Blood Maiden will get you a ton of loot and XP, so it’s worth the effort.
